We are currently on the lookout for 2 PHB Nurse's for roles based in the Yorkshire areas for an initial 3 month contract with a view to extend. As the PHB Nurse, you'll be required to promote PHBs within the community and support the on-going development of the Personal Health Budgets. This role will also include assisting with the introduction of Direct Payments for Health Care and the overall care management of clients.

Key aspects of role:

* Lead the development of a workforce plan that places personal health budgets at the heart of service delivery and links with the wider integration agenda.

* Ensure monitoring tools are implemented according to CHC procedures, specifically use of NHS England CHC Operating Model and associated Assurance Framework.

* Assist in the development of policies and procedures, as requested, which will support the overall delivery of Personal Health Budgets within the CCG.

* To support and contribute to the ongoing development of the service identifying areas of practice that requires improvement.
