Job Summary

*To arrange, procure & monitor individual placements in relation to Personal Health Budgets (PHB) for NHS Continuing Healthcare.

*A key part of this role is to lead on providing clinical leadership and professional expertise. To ensure each package of care delivers high quality care representing value for money within agreed available funding.

*To act as clinical lead for NHS Continuing Care, acting as a specialist advisory role to health, local authority & independent sector staff on NHS CHC PHB related issues. Influencing best practice by providing professional nursing advice on a range of physical health nursing issues to ensure patients health care needs are met appropriately and effectively, whilst achieving outcomes in a timely and appropriate manner.

*Contribute to development of training programmes and delivery plans to facilitate others learning.

*The post will involve travel across the Birmingham and Solihull area.

Key Duties and Responsibilities

*To ensure All Age Continuing Healthcare guidance is followed and that services are delivered to maximise patient safety.

*To support in the development across the organisation in the personalisation agenda and to support the pathway delivery for Personal Health Budgets within Clinical Services

*To support the Personal Health Budget department in working towards the Directorate's goals and objectives

*Required to maintain constructive relationships with a broad range of internal and external stakeholders.

*Monitor and support the provision of clinical supervision to PHB team, ensuring adequate training resources are available to support revalidation and continuing professional development.

*Participate in the development of working practices, processes and protocols to facilitate timely discharge from hospital, prevention of admission to acute settings and reduce the use of out of area placements.

*To respond to complaints within the area of relevance to the post and other areas as required or in the absence of other senior clinicians.

*Responsible for ensuring assessment, case management and review systems are in place for PHB patients in receipt of complex care

*Provide line management support to the PHB nurse assessors, undertaking assessment, clinical case management and review of clients as appropriate.

*Participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice and expertise.

*Contribute to the development of data collection systems that will provide accurate and timely data.

* Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.

*Contribute to corporate business returns, including planning and workforce returns.

*Oversight of all aspects of allocated budgets including PHB's.

*Contribute to performance improvement, taking a lead for identified areas where agreed.

*Develop policies and procedures in own work function with an impact on the wider organisation, as required.
