An NHS CCG is looking for an interim Band 7 CHC Practitioner to support their new Referrals Team.

You will be responsible for the day to day management of a small team of CHC Specialists Nurses and Administrators who manage incoming referrals to the team. These include Checklists, Fast Tracks and D2A Assessments. You will be tasked with insuring the CHC process is completed within the mandated 28 days.

If you have experience working within CHC at a Band 7 level this could be an excellent opportunity for you.

The role is office based, Monday - Friday and is offering a rate of £340 per day inside IR35.
