CHC Social Worker

We are currently working with an NHS organisation who are looking for a Social Worker with Continuing Healthcare to join their team during a busy time.

The role will be for an initial 2 months and you will be part of a well established CHC team and helping them keep in line with the National Framework for Continuing Healthcare and the 28 day turnaround .

The Role:

* Working as part of the MDT to support with Continuing Healthcare assessments

* Working in a locality working in the community so a car will be necessary

* Have a working knowledge of the October 2018 update to the National Framework for Continuing Healthcare
